I am a breast health navigator / CBCN and have been asked to put together a proposed plan to meet with all of our oncology inpatient new consults before their discharge and initial visit in the cancer center.

 

It has been requested that I will see all cancer types in addition to my work in the Women’s Center/ Breast surgeons’ office and time in the cancer center, do discharge teaching, provide educational materials, and meet their emotional / physical needs (and any other service I can think of).  If this was a breast cancer patient I could do this, although I never see them initially as inpatients.

 

Have any of you written such a proposed plan and could you please share it?  It is almost like I have been asked to take over for the floor nurses/ case managers.  I will appreciate any ideas / thoughts you have so I can write this proposal of my “new duties”.
